Description
The Southern Provincial Health Office under the US President’s Emergency Plan for AIDS Relief (PEPFAR) Project through a Cooperative Agreement (CoAg) with the US Centers for Disease Control and Prevention (CDC), is seeking for eligible, self-motivated and qualified applicants for the positions listed below for the various levels (Provincial, District, Facility and Community)
(B) DISTRICT LEVEL
JOB TITLE: COMMUNITY COORDINATORS (06 POSITIONS)
Purpose: The jobholder will provide expert technical oversight and support at Site, and community levels as needed to optimize demand for HIV Prevention Services in order to meet targets. The jobholder will be responsible for finding creative solutions to challenges identified in health promotion; and Provide targeted and data-driven interventions aimed at reaching out to unserved populations (children, Adolescent girls and young women (AGYW), Key populations, and adult men) with HIV prevention, care and treatment services
Responsibilities:
- Implementing creative and aggressive data-driven promotion programs/activities at the facility aimed at creating demand for HIV Prevention, care and treatment services;
- Quantification of populations classified as “Populations at high risk” (FSW, MSM, IDU and prisoners), and priority populations focusing on Adolescent Girls and Young Women (AGYW) and devise age specific health promotion programs/activities;
- Coordinating community HIV prevention, care and treatment services in collaboration with Community Implementing Partners in order to accelerate HIV Case Identification, linkage to care, maximal retention, and viral load suppression
- Coordinate Community Adherence Support Groups (CAG) in the District by ensuring that all support districts and facilities have targets and are implementing and scaling up;
- Coordinate activities of the community sub-grantee (DAPP) in assigned districts;
- Manage data and provide monthly reports on the number of patients served under the CAG model; and
- Perform other duties assigned by supervisors at district and provincial levels
Qualifications:
- Grade Twelve (12) Certificate with Five (5) O’ Levels
- Bachelor’s degree/Diploma in Social Sciences.
- Preferred experience of 3-5 years working in HIV setting at community level Communities.
- Outstanding written communication and presentation skills.
- Strong experience preparing reports.
- Friendly, enthusiastic and positive attitude.
- Strong knowledge of social media and other basic social marketing platforms.
